Nursing Home Outreach and Transition |
| Many non-elderly people with disabilities find themselves in nursing facilities due to inaccessible housing or loss of a caregiver. It is very difficult for them to make the arrangements needed to move back to their communities, especially if they need an accessible place to live. |
| Independence Now can help: Volunteer Peer Mentoring Project: We have a group of trained volunteers, called Peer Mentors who are matched with people who want to leave nursing facilities. All of these volunteers are people with disabilities. Many of them lived in nursing facilities at one time. We have a group of trained volunteers, called Peer Mentors who are matched with people who want to leave nursing facilities. All of these volunteers are people with disabilities. Many of them lived in nursing facilities at one time. Transition Services: Independence Now staff assist people in nursing facilities subsidized housing, personal assistance services and other services they may need in order to return to the community. Sometimes our staff can find funding to pay for equipment that a person needs or for transition costs, such as a security deposit. |
