Bringing Independence to Nursing Home Residents Through Money Follows the Person

Independence Now conducted outreach at Heartland Health Center in Hyattsville, MD through the Money Follows the Person (MFP) program. The goal of the program is to assist Montgomery and Prince George’s Counties nursing home residents who choose to transition to a community-based setting. Team members (pictured) Mike Saunders, MFP Director, and Damon Walker, MFP Peer Support Counselor, were joined by Noelle Valla, Supports Planner, who discussed case management services that are offered to Medicaid-eligible beneficiaries. The Heartland visit also included fun shaving cream art instruction led by Damon and loved by the residents.

Through our Money Follows the Person program, Independence Now provides hands-on support to nursing home residents who want to regain their independence in the community. Our team works one-on-one with individuals to navigate the transition process, ensuring they have access to housing, healthcare, personal assistance services, and other essential resources.
